 چکیده

با شیوع سریع COVID-19 کرونا ویروس ۲۰۱۹ در سراسر جهان، میزان بروز آن در کشورهای مختلف بسیار متفاوت است. این اختلافات این سؤال را به وجود می‌آورد که آیا کشورهایی که شیوع کمتری دارند، دارای ویژگی‌های مشترک پزشکی هستند که می‌تواند نه تنها برای توضیح این میزان پایین بودن بیماری بلکه برای راهنمایی برای درمان‌های بالقوه در سایر نقاط نیز مورد استفاده قرار گیرد. چنین درمانی اگر بتواند به عنوان پیشگیری کننده در برابر انتقال SARS-CoV-2 مورد استفاده قرار گیرد، بسیار ارزشمند خواهد بود، در نتیجه سرعت گسترش بیماری را کاهش می‌دهد. در اینجا، ما نشان می‌دهیم که در کشورهایی که داروهای پیشگیری کننده از جمله ایورمکتین دارند، شیوع COVID-19 به طور قابل توجهی پایین‌تر است. استفاده پیشگیرانه از ایورمکتین در برابر عفونت‌های انگلی در آفریقا شایع است و از این رو نشان می‌دهیم که همبستگی گزارش شده هم در مقایسه با کشورهای آفریقایی و هم در زمینه‌های جهانی بسیار چشمگیر است. این ممکن است به توانایی ایورمکتین در مهار تکثیر SARS-CoV-2 مرتبط باشد، که احتمالاً منجر به پایین آمدن میزان شیوع می‌شود. با این حال، برای توضیح ماندگاری چنین اثر مهاری پس از کاهش سطح سرمی ایورمکتین، باید مسیرهای دیگری وجود داشته باشد. پیشنهاد می‌شود که ایورمکتین از نظر کاربرد پیشگیرانه ارزیابی شود تا به واکسیناسیون ایمن و مؤثر کمک کند.

Title: A COVID-19 prophylaxis? Lower incidence associated with prophylactic administration of ivermectin

Abstract

 Objectives: Coronavirus disease 2019 (COVID-19) has become a worldwide pandemic. However, the hazard to newborns in pregnancy remains controversial. The aim of this study was to investigate the vertical transmission of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) from mother to child and developmental toxicity in the fetus. Methods: All clinical information was recorded on 22 neonates born to mothers with confirmed COVID-19 pneumonia in Tongji Hospital. As COVID-19 (coronavirus disease 2019) continues to rapidly spread throughout the world, the incidence varies greatly among different countries. These differences raise the question whether nations with a lower incidence share any medical commonalities that could be used not only to explain that lower incidence but also to provide guidance for potential treatments elsewhere. Such a treatment would be particularly valuable if it could be used as a prophylactic against SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) transmission, thereby effectively slowing the spread of the disease while we await the wide availability of safe and effective vaccines. Here, we show that countries with routine mass drug administration of prophylactic chemotherapy including ivermectin have a significantly lower incidence of COVID-19. Prophylactic use of ivermectin against parasitic infections is most common in Africa and we hence show that the reported correlation is highly significant both when compared among African nations as well as in a worldwide context. We surmise that this may be connected to ivermectin’s ability to inhibit SARS-CoV-2 replication, which likely leads to lower infection rates. However, other pathways must exist to explain the persistence of such an inhibitory effect after serum levels of ivermectin have declined. It is suggested that ivermectin be evaluated for potential off-label prophylactic use in certain cases to help bridge the time until a safe and effective vaccine becomes available. Conclusions: This study was the first to discover that COVID-19 infection in the third trimester of pregnancy could cause fetal kidney developmental injury, as indicated by increased cystatin C and β2- microglobulin in all neonates. Furthermore, there is the possibility of maternal fetal transmission of SARSCoV-2.
